Provide a thorough description of your dog's behaviors, good and bad, ideally in chronological order with approximate dates of when they occured.
Include your dog's past illnesses or injuries, any OTC or prescription medications your dog takes and why, your current veterinarian, and if your dog is up to date or behind on check ups and vaccinations.

Include what you have done to help adjust your dog's behaviors.

Include when fed, how, and brand of food.

Include ages of family members, energy level of the home and neighborhood, and other pets in the home.
